Leadership Reporting Forms

The Leadership Report and Lay Delegate Certification Form are distributed annually to collect updated church, clergy and lay leadership contact information.  Congregations are asked to send updated information requested in the forms to the Office of Convention and Council within 10 days of their annual meeting as required by diocesan canons and no later than Friday, March 26.  Questions may be directed to Chelsea Smith, Assistant for Governance and Administration, at csmith@diomass.org or 617-482-4826, ext. 406.

There are three simplified components to the online reporting process for 2021:

1.  Use the online 2021 Congregation Leadership Form to report Church Contact Information; Licensed Lay Ministers; and Safe Church Audit.  Click here to access the Congregation Leadership Form.

2.  Separately, an Excel file will be e-mailed to parish administrators and clergy for updating all other leadership changes.  See the letter attached below for more details.
